Factores asociados a la no adherencia al tratamiento antihipertensivo en pacientes atendidos en el Hospital Regional de Ica 2025

Abstract

Objetivo. Identificar los factores asociados a la no adherencia al tratamiento antihipertensivo en pacientes atendidos en el Hospital Regional de Ica 2025. Metodología. Investigación de tipo observacional, trasversal, prospectiva y analítica en una población de 380 paciente hipertensos de donde se obtuvo un tamaño de muestra de 191 pacientes, a quienes se le aplicó el cuestionario Morisky Green de 8 ítems para medir el grado de adherencia al tratamiento antihipertensivo, el cuestionario de Batalla para medir el grado de conocimiento sobre la hipertensión arterial, y una ficha estructurada para obtener las variables sociodemográficas, los datos se procesaron en el progra estadístico SPSS v29, presentando los resultado0s con un error menor al 5%. Resultados: El 31,9% de los pacientes tienen baja adherencia al tratamiento, 44,5% tienen grado medio y el 23,6% grado de adherencia alto. Los factores asociados a la baja adherencia al tratamiento fueron: Edad de 31 a 59 años (p<0,05) . sexo masculino (p< 0,05), bajo grado de instrucción primaria o secundaria (p< 0,05), tener un estado civil con sin pareja (soltero, viudo, divorciado) (p< 0,05), tener terapia multidroga (p< 0,05), presentar efectos adversos a los medicamentos (p< 0,05) y tener insuficiente nivel de conocimientos sobre la enfermedad (p< 0,05). Conclusiones: Los factores asociados a la no adherencia al tratamiento antihipertensivo son de tipo socio-demográficos, clínicos y de conocimiento sobre la enfermedad.

Objective. To identify the factors associated with non-adherence to antihypertensive treatment in patients treated at the Regional Hospital of Ica in 2025. Methodology. This is an observational, cross-sectional, prospective, and analytical study in a population of 380 hypertensive patients, from which a sample size of 191 patients was obtained. The Morisky Green questionnaire of 8 items was applied to measure the degree of adherence to antihypertensive treatment, the Batalla questionnaire to measure the degree of knowledge about arterial hypertension, and a structured form to obtain sociodemographic variables. The data were processed in the SPSS v29 statistical program, presenting the results with an error of less than 5%. Results. 31.9% of patients have low adherence to treatment, 44.5% have a medium degree, and 23.6% have a high degree of adherence. The factors associated with low adherence to treatment were: age between 31 and 59 years (p<0.05), male sex (p<0.05), low level of primary or secondary education (p<0.05), being single (unmarried, widowed, divorced) (p<0.05), receiving multidrug therapy (p<0.05), experiencing adverse effects from medications (p<0.05), and having an insufficient level of knowledge about the disease (p<0.05). Conclusions The factors associated with non-adherence to antihypertensive treatment are sociodemographic, clinical, and knowledge-related.
