Tratamiento farmacoterapéutico en pacientes adultos mayores con infecciones genitourinarias: tasas de cumplimiento y complicaciones asociadas - Centro de Salud de Acomayo

Abstract

Objetivo: Analizar el tratamiento farmacoterapéutico en infecciones genitourinarias y determinar las tasas de cumplimiento y complicaciones asociadas en pacientes adultos mayores atendidos en el Centro de Salud de Acomayo. Metodología: Estudio aplicado, descriptivo, transversal y diseño no experimental. La población estuvo conformada por adultos mayores diagnosticados con infección genitourinaria en el Centro de Salud de Acomayo; la muestra fue de 75 pacientes, seleccionados mediante fórmula de muestreo para población finita. Se aplicó una encuesta estructurada validada por juicio de expertos y con confiabilidad test–retest (r = 0,73). Resultados: El 84,0% de los pacientes fue de sexo femenino. La edad media de 70,16 ± 7,24 años. Las comorbilidades más frecuentes fueron diabetes mellitus tipo 2 (26,7%) e hipertensión arterial (21,3%). Los antibióticos más prescritos fueron ciprofloxacino (30,7%), gentamicina (24,0%) y amikacina (16,0%). El 93,3% recibió los medicamentos directamente del centro de salud. Conclusiones: Respecto a la adherencia terapéutica, el 66,7% mostró cumplimiento total, el 24,0% cumplimiento parcial y el 9,3% no cumplimiento; las principales causas de incumplimiento fueron olvido (18,7%) y efectos secundarios (5,3%). Solo el 14,7% consultó dudas con un profesional de salud y el 12,0% utilizó recordatorios. El 13,3% reportó efectos adversos leves (dolor en sitio de aplicación y náuseas), sin registrarse complicaciones graves.

Objective: To analyze the pharmacotherapeutic treatment of genitourinary infections and determine adherence rates and associated complications in elderly patients treated at the Acomayo Health Center. Methodology: Applied, descriptive, cross-sectional, non-experimental study. The population consisted of elderly patients diagnosed with genitourinary infection at the Acomayo Health Center; the sample included 75 patients selected using a finite population sampling formula. A structured questionnaire, validated by expert judgment and with test–retest reliability (r = 0.73), was applied. Results: A total of 84.0% of the patients were female, with a mean age of 70.16 ± 7.24 years. The most frequent comorbidities were type 2 diabetes mellitus (26.7%) and arterial hypertension (21.3%). The most commonly prescribed antibiotics were ciprofloxacin (30.7%), gentamicin (24.0%), and amikacin (16.0%). Additionally, 93.3% of patients received their medications directly from the health center. Conclusions: Regarding therapeutic adherence, 66.7% of patients showed full compliance, 24.0% partial compliance, and 9.3% non-compliance. The main reasons for non-adherence were forgetfulness (18.7%) and side effects (5.3%). Only 14.7% of participants sought professional advice, and 12.0% used reminders. Adverse effects were mild in 13.3% of cases (pain at the injection site and nausea), with no severe complications reported.
