Bajo Peso Al Nacer Y Sus Factores De Riesgo Asociados: Estudio De Casos Y Controles En Un Hospital Nacional Del Perú

Abstract

Objetivo: Determinar factores maternos, obstétricos y perinatales asociados a bajo peso al nacer (BPN) en recién nacidos del Hospital Santa María del Socorro, 2020–2025 (Ica, Perú). Métodos: Estudio observacional analítico retrospectivo de casos y controles sin apareamiento (1:2). Se incluyeron 129 recién nacidos: 43 casos (BPN <2500 g) y 86 controles (≥2500 g), a partir de historias clínicas y registros institucionales. Se realizó análisis descriptivo, bivariado (OR crudo) y regresión logística parsimoniosa. Resultados: No hubo asociación significativa para hipertensión gestacional (ORc 1.35; IC95% 0.22–8.40; p=1.0) ni anemia gestacional (ORc 1.64; IC95% 0.71–3.76; p=0.278). Se asociaron en bivariados controles prenatales inadecuados <6 (ORc 3.53; IC95% 1.59–7.86), prematuridad <37 semanas (ORc 7.94; IC95% 2.38–26.47) e instrucción baja (ORc 8.17; IC95% 1.62–41.24); los abortos previos mostraron asociación inversa (ORc 0.32; IC95% 0.11–0.91). En el multivariado, persistieron como predictores independientes la prematuridad (ORa 6.93; IC95% 1.83–26.32) y el control prenatal insuficiente (ORa 3.13; IC95% 1.30–7.53). Conclusiones: El BPN se asoció principalmente con prematuridad y control prenatal insuficiente, mientras que hipertensión y anemia no se relacionaron en esta muestra.

Methods: Retrospective analytical unmatched case–control study (1:2). We included 129 newborns: 43 cases (LBW <2500 g) and 86 controls (≥2500 g), using medical records and institutional registries. Descriptive, bivariate (crude OR), and parsimonious multivariable logistic regression analyses were performed. Results: Gestational hypertension (cOR 1.35; 95%CI 0.22–8.40; p=1.0) and gestational anemia (cOR 1.64; 95%CI 0.71–3.76; p=0.278) were not significantly associated with LBW. Significant bivariate associations were found for inadequate prenatal care <6 visits (cOR 3.53; 95%CI 1.59–7.86), preterm birth <37 weeks (cOR 7.94; 95%CI 2.38–26.47), and low maternal education (cOR 8.17; 95%CI 1.62–41.24); previous abortions showed an inverse association (cOR 0.32; 95%CI 0.11–0.91). In multivariable analysis, preterm birth (aOR 6.93; 95%CI 1.83–26.32) and inadequate prenatal care (aOR 3.13; 95%CI 1.30–7.53) remained independent predictors. Conclusions: LBW was mainly associated with prematurity and inadequate prenatal care, while hypertension and anemia were not associated in this sample.
