Prevalencia de hipoglucemias inadvertidas según Test de Clarke y su relación con el cumplimiento terapéutico en usuarios diabéticos: Centro de Salud San Juan Bautista, 2025

Abstract

Objetivo: Determinar la prevalencia de hipoglucemias inadvertidas según Test de Clarke y su relación con el cumplimiento terapéutico en usuarios diabéticos: Centro de Salud San Juan Bautista, 2025. Métodos: Estudio aplicado, de nivel descriptivo y diseño observacional transversal. La población estuvo conformada por usuarios diabéticos atendidos en el Centro de Salud San Juan Bautista. La muestra incluyó a 52 pacientes seleccionados mediante muestreo no probabilístico por conveniencia. La percepción de hipoglucemia se evaluó mediante el Test de Clarke y el cumplimiento terapéutico mediante el cuestionario SMAQ. Los datos fueron analizados mediante estadística descriptiva y prueba de chi-cuadrado, considerando un nivel de significancia de p < 0,05. Resultados: La prevalencia de hipoglucemias inadvertidas fue del 17.3%. El 36.5% fue clasificado como cumplidor terapéutico. La población estuvo conformada mayoritariamente por personas de sexo femenino (69.2%), con una edad media de 55.17 años, predominando el diagnóstico de diabetes mellitus tipo 2 (92.3%).No se evidenció una relación estadísticamente significativa entre la presencia de hipoglucemias inadvertidas y el cumplimiento terapéutico (χ² = 0,2934; p = 0,588). Conclusiones: Las hipoglucemias inadvertidas constituyen un problema clínico relevante en el primer nivel de atención. El incumplimiento terapéutico fue elevado en la población estudiada; sin embargo, no se encontró asociación significativa entre ambas variables. Se resalta la utilidad del Test de Clarke como herramienta de tamizaje y la necesidad de fortalecer el seguimiento farmacoterapéutico en pacientes diabéticos

Objective: To determine the prevalence of impaired hypoglycemia awareness according to the Clarke Test and its relationship with therapeutic adherence among diabetic patients attending the San Juan Bautista Health Center, 2025. Methods: An applied study with a descriptive level and a cross-sectional observational design. The population consisted of diabetic patients attending the San Juan Bautista Health Center. The sample included 52 patients selected through non-probability convenience sampling. Hypoglycemia awareness was assessed using the Clarke Test, and therapeutic adherence was evaluated using the SMAQ questionnaire. Data were analyzed using descriptive statistics and the chi-square test, considering a significance level of p < 0.05. Results: The prevalence of impaired hypoglycemia awareness was 17.3%. A total of 36.5% of participants were classified as therapeutically adherent. The population was predominantly female (69.2%), with a mean age of 55.17 years, and type 2 diabetes mellitus was the most frequent diagnosis (92.3%). No statistically significant association was found between impaired hypoglycemia awareness and therapeutic adherence (χ² = 0.2934; p = 0.588). Conclusions: Impaired hypoglycemia awareness represents a clinically relevant problem in primary health care. Therapeutic non-adherence was high in the studied population; however, no significant association was found between the two variables. The usefulness of the Clarke Test as a screening tool is highlighted, as well as the need to strengthen pharmacotherapeutic follow-up in diabetic patients.
